A comparison of different exercise intensities for improving bone mineral density in postmenopausal women with osteoporosis: A systematic review and meta-analysis.

Abstract:OBJECTIVE: This study aimed to compare the effects of moderate- and high-intensity resistance and impact training (MiRIT and HiRIT, respectively) on changes in bone mineral density (BMD) in postmenopausal women with osteoporosis. METHODS: Randomized controlled trials that compared the intervention effects of MiRIT and HiRIT were used as selection criteria to assess study patients with osteoporosis or an osteoporotic condition. Database searches were conducted on August 25, 2022, using CENTRAL, PubMed, CINAHL Web of Science, EMBASE, and MEDLINE. A risk of bias assessment was performed using Revised Cochrane risk of bias tool for the assessment of randomized controlled trials. Point estimates and 95 % confidence intervals of change in BMD derived using dual-energy X-ray absorptiometry were collected as outcomes, and a meta-analysis was performed using the amount of change in BMD before and after the intervention. Adverse event data were also collected. RESULTS: The search yielded six studies (391 patients, mean age 53-65 years) that met the inclusion criteria. The intervention duration ranged from 24 weeks to 13 months. Compared with the MiRIT group, the HiRIT group showed significantly improved BMD of the lumbar spine (standardized mean difference 2.37 [0.10-4.65]). However, a high degree of heterogeneity was observed for three studies (154 patients, I2 = 98 %). Almost all studies reported minimal adverse events. The certainty of evidence was extremely low because of the risk of bias, inconsistency among studies, and imprecision in terms of sample size. CONCLUSION: Postmenopausal women with osteoporosis may achieve more significantly improved lumbar spine BMD with HiRIT than with MiRIT.


Effectiveness of a supervised rehabilitation compared with a home-based rehabilitation following anterior cruciate ligament reconstruction: A systematic review and meta-analysis

Abstract:OBJECTIVE: This systematic review aimed to compare the effectiveness of supervised rehabilitation with regard to knee function with that of home-based rehabilitation in patients undergoing anterior cruciate ligament reconstruction (ACLR). METHODS: The databases searched were: the Cochrane Central Register of Controlled Trials (Central), EMBASE, MEDLINE (via Ovid) and PEDro. All randomized controlled trials comparing supervised rehabilitation (SVR) with home-based rehabilitation (HBR) following ACLR were included. Two reviewers evaluated the study quality using the Cochrane Risk of Bias Assessment (RoB 2.0) tool. Estimates are presented as standardized mean differences (SMD) with 95% confidence intervals (CIs). The quality of evidence was assessed using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach. RESULTS: A total of nine studies met the inclusion criteria, and five studies were included in the meta-analysis. The key outcomes analyzed were self-reported knee function and knee muscle strength. Across all comparisons, there was very low-quality evidence of no significant difference between the SVR and HBR groups at 24 weeks. CONCLUSIONS: The limited evidence available does not suggest that SVR results in superior outcomes than HBR in patients with ACLR. Additional studies are needed to clarify whether patient characteristics and study protocols with longer interventions effect the results.


Effect of physical therapy on the flexibility of the infrapatellar fat pad: A single-blind randomised controlled trial

Abstract:The infrapatellar fat pad plays a biomechanical role in the knee joint. After knee injury or surgery, its dynamics decrease because of an inflammatory response. Physical therapy might be one of the valuable treatments for the recovery of knee joint mobility. This study aimed to evaluate the immediate effect of physical therapy on the dynamics of the infrapatellar fat pad in healthy participants using ultrasonography. In this prospective, single-blind, randomised controlled trial, 64 healthy young participants were enrolled and randomly assigned to one of the following three interventions: manual therapy, hot pack treatment, and control. Ultrasound images of the infrapatellar fat pad were obtained before and after the intervention. The thickness change ratio of the infrapatellar fat pad was calculated to compare the changes between and within groups before and after the intervention. No significant inter-group differences were observed. The effect sizes were relatively small. Manual therapy or hot pack intervention might not have an immediate effect on infrapatellar fat pad flexibility in healthy participants. Thus, it is necessary to consider more intensive treatments to change the dynamics of the infrapatellar fat pad.


Randomised controlled trial for evaluation of an ultrasound-guided palpation intervention for palpation skill training

Abstract:AbstractAlthough there are increasing reports on the usefulness of sonopalpation with ultrasound imaging, many previous studies have reported interventions without a control group. This single-blind, parallel-group randomised controlled trial aimed to determine whether educational instruction with sonopalpation for physical therapy students has a more superior effect on skill improvement than traditional instruction without ultrasonography. Twenty-nine physical therapy students participated in the study and were randomised using block randomisation into an ultrasound imaging group (n = 15) and a control group (n = 14). Subsequently, they underwent three training sessions focusing on the shoulder joint. Participants underwent a scoring assessment of their palpation skills at pre-intervention, post-intervention, and follow-up 3 months after training. The raters were blinded to the subjects’ group. The Friedman and Mann–Whitney U tests were used for data analysis. The intervention group showed a significant increase in scores at post-intervention and the 3-month follow-up; the effect sizes were large (0.849 and 0.849, respectively). A comparison of the scores at different time points after the intervention at the 3-month follow-up revealed no significant difference between the groups. Education using ultrasound imaging may be non-inferior to education without it; nevertheless, further studies are needed to demonstrate superiority.


Usefulness of ultrasonography for dynamic evaluation of medial meniscus hoop function in early knee osteoarthritis

Abstract:AbstractThis study aimed to evaluate the dynamics of the medial meniscus during knee flexion–extension by ultrasonography and compare them with MRI findings to confirm the usefulness of ultrasonography for evaluating early knee osteoarthritis (KOA). In total, 100 patients were diagnosed with early KOA using clinical and radiographical findings. Dynamic ultrasonographic evaluation and MRI were performed in all patients. Medial meniscal extrusion (MME) and medial meniscal tears were evaluated via ultrasonography and MRI. Abnormal MME was defined as MME > 2 mm on ultrasonography during knee extension. Patients with abnormal MME were divided into two groups: a decrease group (group D) and a non-decrease group (group N). Age, sex, absence or type of meniscus tear, and MME were compared between the two groups. Of the 100 patients, 75 demonstrated MME > 2 mm at knee extension. MME at all assessment positions using ultrasonography and MRI were significantly greater in group N (n = 34) than that in group D (n = 41). Medial meniscus posterior root tears or radial tears were observed in most cases in group N. A lack of decrease in MME from 0° to 90° of flexion on ultrasonography was a characteristic finding in patients with a loss of meniscal hoop function.


Flexibility of infrapatellar fat pad affecting anterior knee pain 6 months after anterior cruciate ligament reconstruction with hamstring autograft

Abstract:This study aimed to identify factors affecting anterior knee pain (AKP) after anterior cruciate ligament reconstruction (ACLR) with hamstring tendon autograft using ultrasonography. Forty-two patients were evaluated by ultrasound, 6 months after ACLR. The thickness of the superficial part of the infrapatellar fat pad was measured, as well as the thickness change ratio between the two angles. Color Doppler evaluated the rate of blood flow in the fat pad. AKP was assessed with the Kujala Scale. The correlations between AKP and age, body mass index, the thickness change ratio, and the grade of increased blood flow were examined. Independent variables showing significant correlations with AKP were used for multiple linear regression analysis. There were significant correlations between AKP and age (r = - 0.68), body mass index (r = - 0.37), the thickness change ratio of the fat pad (r = 0.73) and the grade of increased blood flow (r = - 0.42), respectively. Age and the thickness change ratio of the fat pad affected the AKP score (R2 = 0.56). After ACLR, older age and a decrease in the thickness change ratio of the superficial area of the infrapatellar fat pad appear to affect post-operative AKP after 6 months.

Abstract:[Purpose] Increased plantar pressure during walking is a risk factor for foot ulcers because of reduced range of motion at the ankle and first metatarsophalangeal joints. However, the range of motion in patients undergoing hemodialysis has not yet been determined. A cross-sectional study was performed to investigate the factors affecting the range of motion of the ankle and first metatarsophalangeal joints in patients undergoing hemodialysis who walk daily. [Subjects and Methods] Seventy feet of 35 patients receiving hemodialysis therapy were examined. Measurements included the passive range of motion of plantar flexion and dorsiflexion of the ankle joint, and flexion and extension of the first metatarsophalangeal joint. [Results] Hemodialysis duration was not associated with ankle and first metatarsophalangeal joint range of motion in patients undergoing hemodialysis. Diabetes duration was significantly associated with limited ankle joint mobility. Finally, blood hemoglobin levels, body mass index, and age were associated with first metatarsophalangeal joint range of motion. [Conclusion] The present study identified age, diabetes, and decreased physical activity, but not hemodialysis duration, to be risk factors for limited joint mobility of the ankle and first metatarsophalangeal joints in patients undergoing hemodialysis.


地域包括ケア病棟において患者の自宅退院に影響を与える要因

Abstract:在宅から地域包括ケア病棟に入院し退院先が施設・療養型病院となった患者の要因を明らかにするため、2014年10月〜2015年6月に当院の地域包括ケア病棟を退院した122名を対象とし、自宅退院群(89名)と施設・療養型病院への退院・転院群(33名)に分けて諸データの比較検討を行った。調査項目は「年齢」「性別」「在院日数」「入院科」「患者意欲」「情緒・気分」「手段的ADL」「基本的ADL」「認知機能」「MMSEスコア」「退院時のADL(食事・トイレ・移乗・移動)が自立していたか否か」「独居か否か」「同居家族人数」「高齢者世帯か否か」「同居家族の介護力」などとした。検討の結果、退院先に有意な影響を与えている要因として[MMSE]と[退院時の移動動作]が抽出された。

運動課題の違いがpassive stiffnessの変化に及ぼす影響

Abstract:【はじめに,目的】臨床において,joint stiffnessの変化は様々な影響を及ぼす。関節拘縮や痙縮を有する場合はjoint stiffnessが増大しており,逆にjoint stiffnessの低下はスポーツ分野ではエネルギー損失を増大させる結果,力の伝達効率が悪化しパフォーマンスを低下させる。したがってjoint stiffnessの状態を把握し,その変化を知ることはリハビリテーションにおける治療方針の決定や効果判定を行う際に重要である。joint stiffnessはpassive stiffnessとdynamic stiffnessとに分類され,passive stiffnessは運動履歴によって変化することが知られている。passive stiffnessを規定する因子は筋,腱,結合組織など多岐にわたり,その中でどの要因が影響しているのかは判別が困難である。そこで今回,等速性他動運動および等尺性筋収縮がpassive stiffnessに与える影響を推定筋量率の違いから検討した。【方法】対象は健常男性20名であった。被験者の身体的特性として身長,体重,体脂肪率を測定し推定筋量率を算出,平均値を基準として2群(低値群:A群,高値群:B群)に分け比較した。passive stiffnessの指標としてpendulum test(PDT)時の下腿自由落下角加速度を用いた。2つの加速度計を貼付したプラスチック製の板を被験者の下腿に装着し,等速性他動運動課題と大腿四頭筋(Q),ハムストリングス(H)の最大等尺性収縮課題を行った。それぞれの課題前後で,PDT時の下腿自由落下角加速度を計測し,角加速度増加率(課題後/課題前×100)を算出し検討した。統計分析はSPSS Ver.11を使用して対応のないt testを用いた。【結果】等速性他動運動後の角加速度増加率は,A群108.7±17.9%,B群114.4±27.1%,Q課題でA群は91.9±13.5%,B群78.4±12.3%,H課題でA群99.2±16.3%,B群107.4±20.2%となり,Q課題のB群において有意に低値を示した(P<0.05)。【結論】PDT時,下腿の振り子運動状減衰振動は下腿部にかかる重力,膝関節の粘弾性,QとHの伸張反射により影響を受ける。従って角加速度増加率が100%以上では膝関節の運動抵抗は減少し,逆に100%以下の場合は増大したと捉えることが出来る。本研究ではQ課題後にB群で有意に角加速度増加率が低値を示した。この結果から筋量率が高い方が大腿四頭筋の筋収縮後に運動抵抗の増大を生じることが示唆された。また,等速性他動運動において角加速度増加率は100%以上となり,運動抵抗が減少する傾向を示したが,筋量率による差は有意なものではなかった。これは,下腿自由落下時の減衰振動は下腿の形状や長さ,重量などの影響を受ける。しかし今回は身体の形態的な個体差は考慮しておらず,また被験者は比較的筋量率の高いものが多く,下肢の局所筋量を指標としていなかったことも一因である考えられた。近年PDTにおける個人差軽減のモデルが考案されており,今後は補正を加えた上,幅広い対象で局所筋量を反映したデータによる検討が必要であると思われる。

Effect of lumbar lordosis angle in adolescent baseball players on the development of lumbar spondylolysis v1

Abstract:

AbstractBackground: Lumbar spondylolysis is a stress fracture of the lumbar vertebral arch that occurs frequently in adolescents. Lumbar spondylolysis has a high prevalence in athletes, especially baseball players. When lumbar spondylolysis occurs, restriction of sports activities is inevitable until the bony union is achieved. Therefore, prevention of the onset of lumbar spondylolysis is necessary, and it is necessary to elucidate the risk factors that influence the onset of the disease. An increase in lumbar lordosis angle may influence the development of lumbar spondylolysis because the lumbar lordosis angle increases the compressive stress in the vertebral arch. However, there are no reports on the effect of lumbar lordosis angle and the development of lumbar spondylolysis in adolescent baseball players. Therefore, the purpose of this study was to investigate the effect of lumbar lordosis angle on the development of lumbar spondylolysis in adolescent baseball players. METHODS: Eligible patients were those who visited the orthopedic clinic from January 1, 2018, to October 31, 2021. The selection criteria were male baseball players aged 11-18 years who visited the clinic, and the exclusion criteria were those whose superior endplate of L1 and superior endplate of S1 could not be identified in the MRI images. The existence of development of lumbar spondylolysis, lumbar lordosis angle, age, and pitching experience of the above patients will be assessed based on electronic medical records and imaging findings. Statistical analysis was performed using logistic regression analysis, with the objective variable being the existence of lumbar spondylolysis and the explanatory variables being the lumbar lordosis angle, age, and previous pitching experience.Discussion: This study examines the effect of the lumbar lordosis angle on the development of lumbar spondylolysis in adolescent baseball players. An increase in lumbar lordosis angle may influence the development of lumbar spondylolysis and may be a risk factor for the development of lumbar spondylolysis.

Abstract:オスグッド・シュラッター病や腰椎分離症が問題となる成長期サッカー選手を対象としてジャックナイフストレッチが下肢筋タイトネスとキック動作に与える影響について検討した。Jリーグ下部組織に所属している男児22名(平均年齢12.1±1.1歳)を対象とした。調査開始時に身長(cm)、体重(kg)、長坐位前屈距離(cm)、下肢伸展挙上(SLR)(度)、踵殿間距離:HHD(mm)とインステップキック時の骨盤後傾と膝関節、股関節の角度を計測した。その後、ジャックナイフストレッチを毎日朝と夜に5回ずつ行うように指示し、6ヵ月後に再度同項目を計測した。身長は、154.0±10.4cmから158.1±10.1cm(P=0.477)、体重は45.3±9.3kgから48.0±9.6kg(P=0.439)に変化し、長坐位前屈距離は35.5±7.1cmから42.0±6.0cmに有意に増大した(P=0.004)。また、SLR、HHDそれぞれの左右の平均値は、64.9±6.8度から75.3±7.2度(P<0.001)、121.5±32.6mmから111.1±24.9mm(P=0.008)と変化していた。一方で、インステップキック時に骨盤後傾は増大していた。成長期サッカー選手を対象としてジャックナイフストレッチを6ヵ月行うと下肢筋タイトネスが有意に改善した。一方、キック動作においては臨床的効果を見いだすことはできなかった。(著者抄録)
