Bystander CD8 + T cells may be involved in the acute phase of diffuse alveolar damage

Abstract:Acute respiratory distress syndrome (ARDS) is a serious complication of systemic inflammatory response syndrome, and diffuse alveolar damage (DAD) is a histological manifestation of ARDS. Endothelial cell injury is mainly responsible for ARDS. Many neutrophils and macrophages/monocytes, which are inflammatory cells that play a role in innate immunity, infiltrate the lung tissue in DAD. In recent years, it has become clear that CD8 plays an important role not only in the acquired immune system, but also in the innate immune system. Non-antigen-activated bystander CD8 + T cells express the unique granzyme B (GrB) + /CD25-/programmed cell death-1 (PD-1)-phenotype. The involvement of bystander CD8 + T cells in lung tissue in DAD is an unexplored field. This study aimed to determine whether bystander CD8 is involved in DAD. Twenty-three consecutive autopsy specimens were retrieved from patients with DAD, and the phenotypes of infiltrating lymphocytes in the DAD lesions were evaluated using immunohistochemistry. In most cases, the number of CD8 + T cells was higher than that of CD4 + T cells, and many GrB + cells were also observed. However, the number of CD25 + and PD-1 + cells was low. We conclude that bystander CD8 + T cells may be involved in cell injury during the development of DAD.

An autopsy case of rheumatoid arthritis accompanying T-call large granular lymphocytic leukemia and secondary systemic amyloidosis

Abstract:We encountered an autopsy case of rheumatoid arthritis (RA) accompanying T-cell large granular lymphocytic leukemia (T-LGLL) and secondary systemic amyloidosis. The patient was 78-year-old woman, who had been suffering from RA for 20 years. Thorough examination of neutropenia and proteinuria revealed T-LGLL and secondary systemic amyloidosis 3 years before death. She died for renal failure, and an autopsy was performed. Histopathological examination exhibited amyloid deposition in many organs and the infiltration of T-LGLL in bone marrow. Although both T-LGLL and secondary systemic amyloidosis are known to be complication of RA, synchronous development of T-LGLL and secondary amyloidosis is extremely rare. In addition, we discussed the possible contribution of STAT3 abnormality.

Subsets of infiltrating lymphocytes around spiral arteries with or without remodeling in hypertensive disorders of pregnancy

Abstract:【目的】 妊娠高血圧症候群(hypertensive disorders of pregnancy: HDP)はらせん動脈(spiral artery: SA)リモデリング不全が発症の契機と考えられている.SAにおけるリモデリングの有無で血管周囲の免疫応答の違いを明らかにすることを目的とし検討した. 【方法】 2008年1月から2017年12月の期間に信州大学医学附属病院(当院)でHDPと診断され,出産後に胎盤を病理へ提出された173例を対象とした.対照群16例は,同期間に当院で出産した正常な経過を辿り分娩に至った症例,および病理診断時に異常が指摘されておらず妊娠経過に異常のない35週以降の早産の症例を用いた.胎盤病理標本からSAを同定し,acute atherosis(AA),リモデリングされていない筋性血管(MA),リモデリング血管(RA)に分類した.SA周囲のリンパ球サブセットをCD3, CD8, CD56の免疫組織化学で評価した. 【結果】 HDP症例においてAAは55例(31.8%)で認められた.AAがあるHDP群ではRA,MAに比し,AA周囲にはCD3+/CD8+ T細胞が多く,CD3+/CD8- (CD4) T細胞も多かった.CD56+ NK細胞はRAに比しAA, MAで多かった.対象群では,RAに比しMAではCD3+/CD8- (CD4)T細胞が多く,CD56+ NK細胞には差がなかった.AAのあるHDP群と対象群ではRA周囲のCD3+/CD8- (CD4)T細胞とCD56+ NK細胞には差がなかった. 【結語】 AAのあるHDP群と対象群の両群において,リモデリングされていないMA, AAにはCD3+/CD8- (CD4) T細胞が多くリモデリングの有無で差がみられる.このことからSAのリモデリングにはCD3+/CD8- (CD4) T細胞の関与の可能性が考えられる.
